A co-payment is a charge for healthcare services paid by insured patients. Co-payments are due at the time of service. Because co-payments vary by insurance company, contact your insurance company or refer to your insurance card to verify your co-payment responsibility. Please bring your insurance card and co-payment to your appointment. |

| How do I request a copy of my medical records? To request a copy of your medical records, please call the office at 804.272.2702 to speak with the Medical Records Clerk. What are the fees associated with medical record requests? If you are requesting medical records for your personal files, there is a $10.00 search fee in addition to a $0.50 charge per page for the first 50 pages and a $0.25 charge for every page thereafter. If a request for your medical records is made by a lawyer’s office or insurance company, the same search fee and charges per page apply. If asked to have your medical records mailed, applicable postage will be added. There are no charges associated with medical records requests made by other doctors’ offices with whom we share mutual patient care; or to another doctor’s office to whom we have referred a patient. |

| Can I fill out registration and medical history forms prior to my appointment? Yes! You can complete and submit the Pre-Registration and Patient History forms through the Virginia Diabetes & Endocrinology, P.C. secure patient portal prior to your appointment. You will need to create an account and log in to our secure patient portal in order to access these forms. If you are a new patient and are unable to create an account to complete the online Pre-Registration and Patient History forms, you can print the Patient Registration and Medical History forms and bring the completed forms to your first appointment. You can find the Patient Registration and Medical History forms in the “Registration” section under the “Plan My Appointment” tab. How do I schedule an appointment to see a Virginia Diabetes & Endocrinology, P.C. physician? If you are a new patient, you will need to contact our New Patient Coordinator at 804.272.2702 to schedule your appointment. If you are currently a Virginia Diabetes & Endocrinology, P.C. patient, you may schedule an appointment by calling 804.272.2702 and speaking with one of our appointment schedulers. Our Internal Medicine doctors, Dr. Young and Dr. Sandhu, are currently accepting new primary care patients. Please call the office to inquire about appointment availability with Dr. Young or Dr. Sandhu as your primary care physician. If you wish to schedule an appointment with one of our Endocrinologists, you will need a referral from your Primary Care Provider. What should I bring with me to my appointment? Please bring the following to every appointment: -Insurance Card -Co-payment -Insurance Referrals -List of prescription and non-prescription medications, vitamins, and herbal supplements, including dosages -Copies of medical records and lab/procedure results pertinent to your appointment -Patients with Diabetes should also bring their meters, log books, and food journals to each appointment. If you have had any recent demographic changes (address, phone number, insurance, etc.), please inform the front desk staff when you arrive for your appointment. Will I receive an appointment reminder call? As a courtesy, Virginia Diabetes & Endocrinology, P.C. has an automated system that will call patients two days prior to your scheduled appointment. However, as a patient, it is your responsibility to remember your scheduled appointments. There is a $25.00 fee for missed appointments. How often do patients with Diabetes need to see their physicians? As a standard of practice, patients with Diabetes need to see their physicians every 3-4 months. Do I need to fast for my appointment? The best time to discuss whether or not you should fast for an appointment is with your physician at the office, prior to scheduling your next appointment with the front desk staff and before leaving the office. If you are unsure about whether or not to fast for your appointment, please call the Virginia Diabetes & Endocrinology, P.C. office at 804.272.2702. *If you are a patient with Diabetes and unsure about whether or not you should fast for your appointment, please call the office at 804.272.2702 to speak to your physician/nurse about fasting and how to adjust your Diabetes medication when fasting. |

| What is a Primary Care Provider? A Primary Care Provider (PCP) is a Physician, Physician’s Assistant, or Nurse Practitioner who sees patients for routine wellness visits and to treat common illnesses and injuries in non-emergency situations. Referrals from your Primary Care Provider are necessary if you need to see an Endocrinology specialist at Virginia Diabetes & Endocrinology, P.C.. What is an Internist? Internists are Doctors of Internal Medicine. They specialize in adult medicine and specifically focus on the prevention of injury and illness and the promotion of health. Internists are trained to diagnose and manage multiple, complex diseases in adults. What is an Endocrinologist? An Endocrinologist is a physician specially trained to diagnose and treat disorders of the endocrine system. What is a Nurse Practitioner and how are her services used in your practice? Nurse Practitioners are Registered Nurses with extensive education and training in a medical specialty area. They have earned Master’s degrees, are board certified by the American Nurses Credentialing Center, licensed by their states to practice advanced nursing, and granted prescriptive authority. The education, training, and practice of Nurse Practitioners are often focused on disease prevention, the reduction of health risks, and patient education. Nurse practitioners work closely with doctors and nurses and are able to diagnose and treat common illnesses, help to manage chronic diseases as part of a medical team, and prescribe medications. Your visits to Virginia Diabetes & Endocrinology, P.C., depending on the nature of your medical condition, may alternate between your physician and our Nurse Practitioner to allow access to more frequent treatment and intervention. |
