Patients can complete the Pre-registration and Patient History forms and acknowledge receipt of our Notice of Privacy Practices prior to your appointment by visiting the Virginia Diabetes & Endocrinology, P.C. website at  You will need to create an account and log in to our secure patient portal in order to access these forms.

If you are a new patient and are unable to create an account to complete the online Pre-Registration and Patient History forms, you can print the Patient Registration and Medical History forms below and bring the completed forms to your first appointment.

What to Bring

Please bring the following to your appointment:
  • Your insurance card and co-payment
  • A list of your prescription and non-prescription medications, vitamins, and herbal supplements and dosages for each
  • Copies of your medical records and lab/procedure results pertinent to your appointment
  • Patients with diabetes should bring their meters, blood sugar logs, and food journals

*New patients who have not completed the registration and medical history forms online should plan to arrive 15 minutes early for their appointments.
